the 'export fields' that I designated in my SETUP are not showing up in the exported csv file; am getting the Client, Project, date and duration . . . am not getting client code and task and 'Reference' . . . in particular, how do I get the Reference text to show up in a report??

Did you set the Report "Type" to "Time Sheet"?  That will only give a summary of the duration, by Client, by Project.  If so, make sure to set it back to "Time and Expenses".
